How to Apply (for F1 students)
 
 
There are 7 easy steps to apply:

Step 2: Application form
Click here to fill out the application form.
(If you are planning to study in Hawaii for 2 sessions or mroe, please take page 3 - Health Clearance Requirements to your clinic/hospital with your MMR shot history to create MMR immunization record in English).

the application.
 
Step 3:Application and optional service fees

Submit your credit card information with the application form.

If you donít have a credit card, please contact to the IEP office for payment instructions.

Step 4: Financial capability
 
Provide a current bank statement on bank stationery with your name or family's name. (In English)
Fill in the Financial Statement section on the second page of the application form.

Step 5:Academic transcripts
 
Provide evidence of High School or College Completion
(In English). You should get 2 original copies of your transcripts or graduation certificate. Send only one copy to the IEP. Take the second copy with you to your F1 visa interview.
 
Step 6:Copy of your passport
 
Provide a copy of the front page of your passport (the page with your picture, the expiration date of the passport, and your personal information, including your date of birth, country of birth, nationality, etc.) You should also include a copy of the page(s) where changes have been made. For example, if your name was spelled incorrectly on the main page and a correction was made on the following page, you should include a copy of the second page as well.
